Patient Overview

A 53-year-old male patient presented with a complex medical history including paraplegia, balance difficulties, hammertoe deformity, mild cognitive disability, spinal degeneration, brain atrophy, developmental delays, sleep disturbance, and depression. The patient reported no tobacco, alcohol, or drug use with no documented allergies. Current medications included Lexapro, Bactroban, Carmol 40, and iron supplementation.

Clinical Presentation

The patient had a chronic, non-healing Stage 4 pressure wound on the right buttock that began April 11, 2025 — approximately 8 weeks prior to evaluation.

Failed Conservative Approaches

The wound was managed with standard of care including selective sharp debridements, medihoney, and calcium alginate, alongside other conventional dressings with offloading strategies. After more than four weeks without improvement, advanced therapy using DermaBind full-thickness placental membrane was initiated.

Treatment Protocol

After cleansing, a full-thickness DermaBind placental membrane allograft was applied weekly for ten consecutive applications over approximately 10 weeks.

Outcome

At week 10 reassessment, the provider discontinued treatment and documented 100% re-epithelialization.
